Project Coordinator - GRANT

Title Position Reports to: Director, Institute for Parenting
Project Coordinator: Nassau County Closer to the Crib Program - Grant
Job Summary:
The Project Coordinator provides a variety of functions related to the operation of a new Infant/Early Childhood Mental Health-informed Criminal Court model, Closer to the Crib, started by the Nassau County District Attorney's Office. The model includes a comprehensive relationship-based Mental Health assessment of infants, toddlers, preschoolers and their parent(s) and family needs. This includes the development of an individualized family case plan based on the assessment as well as the procurement and coordination of needed services to promote family stability and infant and early childhood well being.
Responsibilities & Duties
Provide the following services:
Develop and maintain linkages with community organizations and government systems serving 0-5 year olds and families
Screen and assess the development and social emotional status of all 0-5 year olds in the program
Screen and assess caregiver and family needs in the program
Develop individualized family service plans in collaboration with other Institute for Parenting staff and the District Attorney
Provide facilitated referrals; coordinate collateral contacts, and track referral status
Schedule and organize monthly infant/family focused team meetings for each case
Inform all parties involved with the case of meeting schedules
Prepare necessary reports based on assessments and progress
Attend court hearings as requested and needed
Participate in meetings of steering committee
Assist with data collection and participate in program evaluation
Continually scan for organizations that serve infants and families
Cultivate relationships with collaborative partners who provide effective evidence-based services and family supports
Work towards continuous improvement of the Closer to the Crib program by conducting an ongoing informal needs assessment to ensure that the needs of all participants in the program are being met
Provide Leadership and assistance in the overall implementation of:
Clinical assessment and screening for referral and service plan recommendations
Collect data to be examined in aggregate for the overall evaluation of the program
Ensure that all documentation related to the provision of assessments are completed in accordance with established policies and procedures and all records of assessments are accurately maintained in a timely manner
Develop a manual of policies and procedures needed to run the Closer to the Crib program and for potential replication of the program
Ensure ongoing assessments and screenings take place in a timely manner, including initial assessments and 6-month follow-up measures
Score measures and complete reports related to each of these assessments and observations
Develop an Advisory/Services Committee for/with Institute for Parenting staff and Executive DA
Use ongoing assessment/follow-up results to suggest recommend changes in services or treatment plans as indicated
Assist in the development of quality assurance system
Participate in the development of appropriate data collection tools and intake procedures
Work as part of team and handle other program planning, development, administrative, marketing tasks as needed
Perform other job related duties as assigned
REQUIREMENTS:
Excellent interpersonal, written, and clinical assessment, screening and program evaluation skills
Ability to network with appropriate university academic and administrative offices
Ability to network with appropriate community agencies and licensure bodies
Computer system proficiency (word processing, spreadsheets, databases, internet)
Ability to deal courteously and effectively with students, staff, faculty, and clients
Ability to maintain confidentiality
Ability to multi-task while effectively planning and organizing work activities
Ability to work independently, exercising good judgment and reasoning in the performance of evolving job duties, while being an effective team member
Have a strong sense of professional ethics and responsibility
Ability to supervise staff as well as work cooperatively and support team collaboration
Experience with systems change and community building
Working knowledge of Microsoft Word, Excel, PowerPoint
The ability to develop and maintain database
Educational/Experience Requirements:
Master's degree (or higher) in social work, psychology, child development or related field
Minimum of five years' experience working in the infant/family field
Strong background in infant mental health including attachment theory and research
Strong background in infant and early childhood development
Background in trauma and trauma-informed interventions
Knowledge of local infant/family serving systems and organizations including Early Intervention, Early Head Start, home visiting programs
Ability to work collaboratively with a multidisciplinary practitioner group
Ability to engage and support court-involved parents
Knowledge of screening assessment tools for infants and caregivers
Clinical expertise in Infant-Toddler-Parent Mental Health
Strong background in clinical theory and practice
Strong background in program development, implementation and evaluation
Strong background, experience, interest and understanding of research and evidence supported prevention and treatment services
Bilingual Spanish speaking desirable
