SUMMARY

The Physical Therapist - Home Health plans and implements specific treatment programs of principles and practices of physical therapist for adult and geriatric patients. Ensures that treatment programs are appropriate for patients' ages, clinical condition and mental status. Collaborates with interdisciplinary team.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Evaluates patients' functional ability using appropriate tests, measurements, and medical history.


  • Coordinates with the physician regarding the specific physical therapy goals to be included in the patient's medical plan of treatment. Additionally, communicates general status and significant changes in patient's condition to physician and to case manager/home care supervisor or to other team members as necessary.


  • Develops a program of therapeutic exercise and treatment for assigned patients. Determines proper equipment usage, application and body position, adjusting as necessary in the administration of treatments. Modifies plans as necessary.


  • Administers prescribed physical therapy treatments to restore function, relieve pain, and prevent further disease or injury, including manual therapeutic exercises (passive, active, and resistive), muscle re-education, gait training, and rehabilitative techniques, and lumbar/cervical traction and its related exercises, as well as treatment using physical agents(weather, heat, cold, light, equipment) as indicated or prescribed by physician.


  • Assesses patients' home environment for potential home rehabilitation.


  • Discusses realistic therapy goals with patients and families/significant others per HIPAA Regulations.


  • Instructs patients and/or caregiver on home exercise program, proper use of wheelchairs, assistive devices, braces, and prosthetic devices.


  • Recommends modifications or changes in the patient's program based on evaluation of progress. Communicating as necessary with physicians concerning alternatives available.

  • Provides a physical therapy plan in the patient's chart and communicates the plan to other members of the home care team. Documents patient's reactions, progress, therapeutic interventions and plans for reach visit. Also completes and discharges summary forms.

    Education and/or Experience

    Bachelor's degree in Physical Therapy required. One year of related experience required.

    Three years of related experience preferred.

    Computer Skills

    Proficient in Microsoft Office software (Word).

    Certificates, Licenses, Registrations

    Current Illinois license in Physical Therapy. CPR certification. Valid Illinois driver's license.
