SUMMARY
The Physical Therapist I evaluates and treats patients using the principles and practices of physical therapy and in accordance with the patient's age, developmental level, and diagnosis. Collaborates with the interdisciplinary team. License-eligible therapists would perform duties under the supervision of a licensed therapist in accordance with the Illinois Physical Therapy Practice Act.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Evaluates patients, establishes goals, and develops individualized treatment plans as prescribed by a licensed physician/practitioner.
Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient's response.
Provides education to patient and family/caregiver regarding patient's abilities and need for assistance and continued therapeutic intervention.
Recommends modification or changes in the patient's treatment program to the referring physician based on evaluation of progress. Confers as necessary with physicians concerning alternatives available.
Prepares written/computer treatment documentation as required by the profession and the department, such as physician orders, evaluation results, treatment plans, progress reports, and other such reports.

Education and/or Experience
Bachelor's degree is required. Candidates must be graduates of an approved school of Physical Therapy in accordance with the Commission on Accreditation in Physical Therapy Education.
Master's and/or Doctorate Degree in Physical Therapy are preferred.
Computer Skills
Proficient in Microsoft Office software (Word).
Certificates, Licenses, Registrations
Current State of Illinois license in Physical Therapy or License eligible for new graduates. Current CPR certification required.
