CONTEXT OF THE POSITION:

The University of Delaware Nurse Managed Primary Care Center (NMPCC) is a growing interdisciplinary primary care practice housed at the University of Delaware's innovative STAR Campus Health Sciences Complex. The Center offers full-service primary care services for individuals aged 13+, including acute illness/disease care, chronic disease management, complete physicals, women's health services, adult immunizations, and sports physicals for adolescents aged 13+. The Center offers care to the general public in the Newark and surrounding communities, as well as to UD employees. Uniquely, the Center also offers nutrition counseling, health coaching, and exercise counseling services. The Center offers a comprehensive telemedicine Parkinson's Disease Clinic, and completes clearance assessments for individuals interested in participating in various research studies at the University. The NMPCC primary care team is led by Family and Adult Nurse Practitioners. The NMPCC serves as a platform for research and education in the University of Delaware College of Health Sciences. Undergraduate and graduate nursing students as well as other students studying healthcare and human services have learning and clinical experiences in the NMPCC. The NMPCC is undergoing Practice Transformation efforts toward Patient Centered Medical Home Certification, and is preparing to launch integrated behavioral and primary healthcare services. The NMPCC is one of three clinics at the STAR Campus Health Sciences Complexes; the NMPCC team collaborates with the Delaware Physical Therapy Clinic, University of Delaware Speech-Language-Hearing Clinic, and with researchers, faculty and students from a variety of departments and fields.

SUMMARY OF THE POSITION:

The Care Coordinator for the University of Delaware Nurse Managed Primary Care Center (NMPCC) is responsible for developing and providing care coordination services for patients of the NMPCC. In addition to providing comprehensive, person-centered resource navigation, chronic disease management and care coordination support to the NMPCC's diverse patient population, the Care Coordinator is responsible for building a streamlined model for Care Coordination in the NMPCC. In collaboration with colleagues in the College of Health Sciences, School of Nursing and NMPCC, the Care Coordinator will develop efficient and comprehensive methods for supporting the NMPCC patient population, documenting care coordination efforts in the electronic medical record, and, importantly, developing mentorship and training for undergraduate and graduate students to learn and participate in care coordination service delivery. The Care Coordinator will be expected to implement evidence-based practices for care coordination services, align with national and state-wide efforts (e.g. State Innovation Model, Practice Transformation, healthcare value-based payment reform), and to adapt models to meet the unique needs of the NMPCC environment and patient population, as well as the research and education goals of the NMPCC.

The College of Health Sciences seeks a creative, compassionate, and innovative individual to lead this important effort in the NMPCC.

MAJOR RESPONSIBILITIES:

1. Oversees the establishment and implementation of NMPCC care coordination services ensuring compliance with university policies and all professional, state and federal rules and regulations.
2. Provides care coordination services to a diverse patient population; provides support on-site in the NMPCC, via telephone and electronic communication, as well as in community-based settings related to community-based resource navigation and chronic disease management.
3. Participates as a member of the interprofessional healthcare team and serves as the connector between disciplines/providers and the patient/family.
4. Develops clinical and service learning placement opportunities in the NMPCC related to care coordination; trains, precepts and mentors students.
5. Ensures cost-effective, evidence-based care coordination practice, including collaboration on patient care plans, electronic documentation procedures, use of the electronic patient portal for patient engagement, referral management procedures.
6. Engages in continuous assessment and quality improvement for NMPCC programs and practices.
7. Supports the continued development of comprehensive care, integrated behavioral and primary healthcare services, and prevention services in the NMPCC.
8. Develops innovative methods for connecting community-based programs with clinical service delivery, e.g. through the use of telemedicine technology, developing community-based services for the NMPCC, etc.
9. Participates in professional organizations and professional development opportunities to keep abreast of trends and evidence-based practices in care coordination, nurse managed health centers, and Patient Centered Medical Home.

QUALIFICATIONS:

1. Bachelors degree in nursing, social work, or a related profession. Masters degree preferred.
2. Minimum of three years of care coordination, care management or resource navigation service provision. Experience in a primary care or community setting preferred.
3. Experience developing new programs or initiatives preferred.
4. Advanced knowledge of current, evidence-based practice principles, methods and procedures for the provision of care coordination services.
5. Effective interpersonal and communication skills; effective group facilitation skills.
6. Ability to work effectively with a wide range of constituencies in a diverse community.
7. Basic knowledge of computer software, e.g. Word, Excel
8. Experience working with Health Information Technology such as Electronic medical record (EMR) and patient portals preferred.
9. Knowledge of and experience with outcomes measurement, tracking and reporting.
10. Experience providing mentorship and training to students/interns preferred.
11. Ability to set priorities and manage multiple responsibilities effectively; ability to be flexible and work under pressure.
