Physical Requirements Requires handling of light-weight objects up to 10 pounds, or some standing or walking.
Shift Primarily workings days, Mon-Sun, 8a-4:30p with flexibilty to work evenings and holidays as necessary.
Job Summary Provides medical social services to patients and families in cases where specific problems related to environment and/or psychological aspects influence the effective inpatient or outpatient treatment; formulates and implements the patient s discharge plan with an assigned medical service which may be at Chandler or at Good Samaritan. Provides assessments in situations of abuse and neglect of all age groups, provides cse management plans and follow up as needed.
Skills / Knowledge / Abilities All activities involve high levels of concentration and mental attention.

Preferred Education/Experience 1 year social work experience, preferrably in a medical setting
