WELLNESS NURSE LPN or RN 11 pm - 7 am Part Time NEW POND VILLAGE Walpole Monitor the health & well-being of residents by coordinating resident wellbeing visits. Accurately & propmptly transcribe orders & procure medications & resident care equipment as needed. Supervises the accurate delivery of medications to residents. You must have a current state license as a Registered or Practical/Vocational Nurse & a minimum of 1 year experience in assisted living, hospice, home health, acute or long term care environment. CNA 7 am to 3 pm 3 pm to 11 pm Part Time Provide warm & engaging personal care services, assistance, support & companionship. When certified, administer or assist with medication. Assist with creation & implementation of Resident Care Plans.
